When you’re diagnosed with Hodgkin lymphoma, one of the first things your medical team will do is determine your stage. This classification system might sound intimidating, but understanding how staging works and what your stage means can help you make sense of your treatment plan and what to expect moving forward.
Staging answers a specific question: where has the lymphoma spread in your body? It’s not a measure of how “bad” your cancer is or how sick you are. Instead, staging describes the extent and location of lymphoma cells.
The staging system used for Hodgkin lymphoma is called the Lugano classification, based on the older Ann Arbor system. This system divides the body at the diaphragm (the muscle separating your chest from your abdomen) and tracks whether lymphoma appears above it, below it, or on both sides.
Understanding your stage helps your oncology team choose the most effective treatment approach. Treatment intensity is typically based on whether you have limited stage (stages I and II) or advanced stage (stages III and IV) disease, along with other factors that affect prognosis.
Stage I means lymphoma is found in one lymph node region or in one organ outside the lymph system. This is the most localized form of the disease.
Stage II indicates involvement of two or more lymph node regions on the same side of the diaphragm. For example, you might have affected lymph nodes in your neck and chest, both above the diaphragm.
Stage III describes lymphoma in lymph node regions on both sides of the diaphragm. This means lymphoma appears both above and below that dividing line, such as in nodes in your chest and abdomen.
Stage IV represents the most widespread disease, with lymphoma in organs beyond the lymph nodes, such as the liver, bone marrow, or lungs. Multiple areas of organ involvement indicate stage IV disease.

Your stage designation often includes additional letters that provide crucial information about your specific situation.
The A or B designation refers to symptoms. If you haven’t experienced B symptoms, your stage includes an A (for example, stage IIA). B symptoms include unexplained fever above 100.4°F, drenching night sweats, and unexplained weight loss of more than 10% of your body weight in six months. The presence of B symptoms typically indicates more active disease and influences treatment decisions.
The letter E appears when lymphoma extends from lymph nodes into a nearby organ. For instance, if affected chest lymph nodes extend into lung tissue, you might see stage IIE. This direct extension is different from widespread organ involvement seen in stage IV.
The letter X indicates bulky disease, typically defined as a mass larger than 10 centimeters or a chest mass wider than one-third of your chest diameter on imaging. Bulky disease may require more intensive treatment even at earlier stages.
The letter S means the spleen contains lymphoma. Since the spleen is part of the lymph system, its involvement is noted specifically.
Staging requires multiple types of tests to create a complete picture of where lymphoma exists in your body.
Research shows that PET-CT is more accurate than CT scans alone, detecting lymphoma involvement that CT might miss. The PET component identifies areas of high metabolic activity where lymphoma cells are actively growing, while the CT provides detailed anatomical images.
During a PET scan, you receive an injection of radioactive glucose. Cancer cells consume more glucose than normal cells, so they light up on the scan. This allows doctors to see not just enlarged lymph nodes, but any area where lymphoma cells are metabolically active.
Blood tests measure various markers that help assess disease activity and organ function. An elevated sedimentation rate (ESR) can indicate more active disease and is considered a risk factor. Blood work also evaluates liver and kidney function, blood cell counts, and other factors that guide treatment planning.
Bone marrow biopsy was once routine for staging but is no longer standard practice for most Hodgkin lymphoma patients. PET-CT has proven accurate enough to detect bone marrow involvement in most cases, sparing patients this more invasive procedure.
Your doctor will carefully examine all lymph node regions you can feel (neck, underarms, groin) and check your spleen and liver for enlargement.
Rather than treating based strictly on stage numbers, doctors group Hodgkin lymphoma into limited stage and advanced stage categories, each requiring different treatment approaches.
Limited stage generally includes stages I and II without unfavorable risk factors. Treatment typically involves a shorter course of chemotherapy (often 2-4 cycles of ABVD chemotherapy), sometimes followed by radiation therapy to affected areas. The goal is effective treatment with minimal long-term side effects.
Advanced stage includes stages III and IV, plus stage II with certain high-risk features like bulky disease or B symptoms. These patients typically receive more intensive treatment, usually 6 cycles of chemotherapy. According to the National Cancer Institute, radiation may be added for areas of bulky disease or residual abnormalities on post-treatment scans.
Stage alone doesn’t tell the complete story. Doctors also consider other prognostic factors to predict outcomes and tailor treatment intensity.
For limited stage disease, unfavorable risk factors include bulky disease, involvement of three or more lymph node areas, elevated sedimentation rate, B symptoms, and involvement of organs outside lymph nodes. Patients with these factors receive more intensive treatment even though they have early-stage disease.
For advanced stage disease, the International Prognostic Score (IPS) uses seven factors to estimate prognosis: age over 45, male gender, stage IV disease, low albumin level, low hemoglobin, high white blood cell count, and low lymphocyte count. Each factor present adds one point. Higher scores indicate a need for more intensive treatment approaches.
Hodgkin lymphoma is highly curable at all stages. Stage IV doesn’t mean you can’t be cured. Advanced stage disease requires more intensive treatment, but cure rates remain excellent.
Even with stage IV Hodgkin lymphoma, most patients achieve complete remission with modern treatment regimens. The difference between stages is primarily about treatment intensity and duration, not whether cure is possible.
Doctors also monitor how your lymphoma responds to treatment through interim PET scans, usually after 2-4 cycles of chemotherapy. Response to treatment is more predictive of outcome than initial stage. Patients whose PET scans show complete metabolic response early in treatment have excellent prognoses regardless of their initial stage.
Your initial stage influences your long-term surveillance plan after treatment ends. Patients treated for advanced stage disease may require more frequent monitoring or longer follow-up periods.
Survivorship care focuses on watching for potential relapse and managing any late effects from treatment. The specific surveillance schedule depends on your stage, the treatment you received, and your individual risk factors.
Regular follow-up typically includes physical exams, blood work, and periodic imaging. The intensity and frequency of monitoring usually decreases over time as you remain cancer-free.
